Whether you have applied and been denied, are in the middle of an application, or are just beginning, in order to be prepared, we ask that you please review the following information.

General Information

This section will cover the basics.

Please bring your state-issued identification. (A copy of your social security card will suffice.)

Be thinking about a friend or family member who knows about your condition. You’ll be required to provide their name, address, and phone number.

Information about your education history will be asked. You’ll need to provide the highest level of education you completed and dates.

Medical Information

This section of the interview will cover the medical (physical or mental) information as it relates to your disability. It is important to be thorough.

Compile a list of all the physical and mental conditions you suffer from. (This can include: arthritis, depression, anxiety, etc.)

Compile information regarding your doctors.

   We will need to know the name of the doctor, where they practice, and the phone number.

   We will need to know what tests each doctor may have ordered, when, and for what.

   We will need to know what medications each doctor has prescribed, and for what.

Compile information regarding your impatient and outpatient hospital stays.

   We will need to know when you were admitted and for what.

   We will need to know if any tests were administered during your stay.

   We will need to know what treatment you received. 

Work Activity

We will need to determine when you became unable to work, and why.

We will also need to outline work history for the past 15 years. You’ll be asked job title, type of business, and start and end dates, rate of pay, and number of hours worked weekly.
